The Advanced Board and Train Contract is required to be completed, signed and returned before your dog is dropped off for their Advanced Board & Train.

Down the form below in either MS word or PDF.  Please print, complete, sign and send to bark@elenask-9.com or bring with you when you drop off your dog.

IMPORTANT: A printed paper copy of your dog’s current Rabies Vaccination Certificate must be provided at the time of drop-off. Dogs without a current paper copy of their rabies vaccination record cannot be admitted for boarding or Advanced Board & Training.
